Completing a Matching Gift Form

If your company matches donations made to Chrysalis Center, here are two ways to initiate the matching gift contribution:

• If your company uses an online matching gift platform, submit a matching gift verification request through your company’s online portal and select “Chrysalis Center, Inc.”

OR

• Obtain the appropriate matching gift verification form from your personnel/human resources office. Please complete the form and email, mail or fax the form and a copy of your receipt (if available) to:

Note: Please be sure to complete the entire donor portion of the form and to include a copy of your gift receipt (if available). If you would like to designate the matching gift to a specific funding area, please indicate that on the form.
